Married name or maiden name?

I'm guessing it's ok but figured better to be safe and ask...Is it ok to use her married name on all the AOS paperwork?

Also, in the Philippines it's normal to take the maiden last name and use that as a middle name after marriage. Will doing that cause an issue?

Yes, it is fine to use her married name. It is also perfectly acceptable to change her name so that US records use her maiden name as her given middle name. I know a women that changed her name completely after marriage just by putting the name she wanted on the marriage certificate.
