We are filing for our AOS and we were wondering about the medical information needed on the I-693.

My husband received 2 sealed envelopes from the Dutch consulate, one which he handed over at the POE and a white envelope that says "K1 - Applicant copies of medical forms. Retain this sealed envelope for your I-485 application with the USCIS Office. DO NOT OPEN". When trying to read through the envelope we can make out that there is some medical information on these papers.

He also has a seperate copy of his vaccine record DS 3025 which is not sealed.

Now we were wondering which documents need to be filed/send in? Since he has done a full medical prior to moving here, we don't need the I-693 right? And would he be ok if he only sends in the DS-3025 or does he need to send in the sealed envelope? Or does he need to keep the sealed envelope for an interview at some point?

Reading the I-693 instructions I THINK he needs to send in only the DS-3025 form. But we are not sure.

"What if I am a K nonimmigrant visa holder and already had a medical exam overseas?

If you were admitted as a:

a. K-1 fiancee

(1) You are not required to have another medical examination as long as your Form I-485 is filed within one year of your overseas medical examination, and the medical examination did not reveal a Class A medical condition, or if you did have a Class A medical condition, you received a waiver of inadmissibility and have complied with the terms and conditions on the waiver.

(2) Even if a new medical examination is not required, you must still show proof that you complied with the vaccination requirements. If the vaccination record (D 3025) was not properly completed and included as part of the original overseas medical examination report, you will have to have the vaccination report completed by a designated civil surgeon. In this case, you are required to submit the vaccination record and page 1 of Form I-693."

I do not know what is in your white envelope. Not all consulates give such a thing.

Well, when you file for AOS and send in I-485 and I-765 and you don't need a medical because you already had one overseas, what would you have to send to USCIS to prove that?

The DS-3025 is filled out completely and is checked appropriately according to that post.

It says Vaccine History Incomplete but then it's checked he may be eligible for blanker waiver. That's because he did not have a flu shot nor did he have the varicella shot (he had chicken pox as a kid so no need and it's checked appropriately)

If we read it correctly all he needs to send in is a copy of that DS-3025 but we find that odd since that's only a vaccination worksheet and we don't want to risk losing the white envelope (which has medical stuff in it, just not sure what) in case we need it for an interview along the road.

Your K-1 medical infos were forwarded at POE when you entered with the K-1 visa, they were in the big sealed envelope that you handed to the CBP officer. Technically, USCIS should already have all those documents, but sending a photocopy of your DS-3025 is kind of a second proof that you complied with the AOS vaccination requirements. It is not required to do it, but it's better to do it.

K-1 Visa Timeline AOS Timeline

- Aug 31st, 2011 - Mailed I-129F package - May 29th, 2012 - Mailed AOS/EAD/AP package

- Apr 13th, 2012 - Visa received - Aug 24th, 2012 - Green Card received

ROC Timeline

- May 19th, 2014 - Mailed ROC package to CSC

- Aug 8th, 2014 - Green Card received

N-400 Timeline

- Dec 29th, 2021 - Filed online. Got notice that biometrics will be reused.

- Now waiting...

You can send your sealed white envelope too, if you wish. I personally would hang onto it in case it has a copy of your medical paperwork. Sometimes they lose the copies you hand over at your POE.

I had my interview at the consulate in Amsterdam too and was given the same sealed envelope. I sent it together with all the other documents in the 1-485 package without opening the sealed envelope.

AOS JOURNEY

19-04-2013...................AOS package sent.

24-04-2013................... NOA1 received by text and email.

29-04-2013................... NOA1 received by post for AOS and EAD

03-05-2013...................Received biometric appointment notice.

13-05-2013...................Received RFE for AOS by text

20-05-2013................... Biometric done

09-06-2013.................... Sent RFE response

12-06-2013..................... Case updated to Response review

06-07-2013..................... EAD Approved

13-07-2013...................... EAD received in Mail

We will be sending in a copy of the DS-3025 and in case of an RFE we will send the envelope. Thanks all for the information.

Yea, the posters haven't been online in 3 years.  I would do as they said they were doing, send a copy of the DS3025 and keep the sealed envelope in case of RFE.
