What if you have more than 2 ex'es? The form only has room for 2. Attach a separate sheet of paper or another form?

Exactly. Just make sure you make it clear what case it is by using the case number or your social ect.

Turns out the boxes to type information in to in this form are too short for some of the information I need to add. So, I should just make a separate text document label it as a supplemental to the G-325A and fill in the information on the supplemental page and in the boxes on the form itself refer to the supplemental page?

Yes, and besides the form number also state the actual question itself at the top of your supplement sheet. Sign and date it too. Put "SEE ATTACHMENT" in the box for that question.
