we haven't changed her last name at the social security offices after we married, the 90 day window to do so ran out. We are finishing the Aos forms. and on all the forms we put my last name as hers even though it wasnt changed at a ss office. my question is does her last name when she prints and signs it on the forms need to be with her new last name now that she's married, or with her maiden last name listed on her passport.

Also i noticed our marriage certificate we are submitting with the forms does not have her last name changed but it has the grooms last name which is mine.

It doesnt matter! The way to change her name is just bu filling aos with her new name. It has nothing to do with her ssn. Usually k1 visa holder gets their ssn after AoS or before getting married so it doesnt matter. Once she receive her EAD she can go and hace it change. And FYI wedding certificate usually holds her maiden name.

If she will be using your last name from now on, then that is the name you use on all the AOS forms. The name you use on the forms is the name her EAD/AP and green card will come with. The copy of your marriage certificate acts as a legal name change document. Once she gets the EAD/AP in her married name, she can go to the SSA and get her SSN in her married name too.
