Timeline

Hi. Medical booked for july 2nd in london. The receptionist told me I need to download and complete a form to bring with me. Looked on the embassy website but can't find it. Do we know what this form is called and how to find it? Thanks guys. X

The teacher in me wants to tell you (and others) where to find it because people need to realize there are important links not to be overlooked on London's webpages. They tell you on the K1 instruction page to click "medical" in the left hand navigation bar. Once on the medical page, on the right it says:

THE MEDICAL EXAMINATION

Additional information concerning the medical examination including the forms the applicant is required to complete and present to the panel physician on the day of the medical (PDF 343kb).

Further information is also available from the Department of State website

Medical Examination Fees:

15 and over - £227

14 and under - £105

Additional fees may apply if further tests are required. Fees are payable at the time of the medical examination. The fees are payable in cash or by credit card, Visa and MasterCard only, and debit card.

That first sentence is a link to the medical documents you need. It's easy to overlook things, so to all London K1s in general...look left, right, and center for links and read them.

Also, the price has gone up since that page was last updated. It's currently £235 for 15 and over.

* Naturalisation (Citizenship) via Phoenix Lockbox

* San Francisco Field Office:
1st May 2016 - N-400 window opened
20th August 2016 - N-400 filed

26th August 2016 - NOA1
13th September 2016 - Biometrics

12th January 2017 - Biometrics (again)
30th May 2017 - Interview (approved)

Thanks all. God bless the teacer in yu Nich Nick because I'm really rubbish! Trying to do lots of this on smart phones doesn't have quiet the same result. Find the website hard to naivigate at times. I will be delighted when this visa is in my hand. Then I can go to states and fill in a whole load more forms! Haha! Thanks again all.

It's the phone that is rubbish. My iPad always kicks in the mobile version of the London website and it does not show all the links and proper sidebars. I can't find things that I know exactly where they are. You will miss a lot on the mobile version. I have to manually switch to "full site" every time I am trying to give a link to somebody. No way I could have done it on a phone.
