AOS question on part 1

I really need your good input pleasey

on I-864 form I myself a petitioner and the primary sponsor due to don't have enough income so my parent ( mother) will be the co sponsor, plus my employer also included .. I have a question on 1.d. & 1.e from the part one(1) Which of those line will them to check..

1.d. > I am the only joint sponsor

1.e. > I am the __ first ___ second of joint sponsor

and for part two(2) principal immigrant information for alien registration #..what do I put?

On part 3 information of the immigrant that I have sponsored am lil bit confuse

I have three(3) cases in one family which who are 2 minor and my wife.. so should I have to put their names on the family member 1 -6

if it doesn't just mark none? or what is correct.

Last thing I should know is this form I-864 still valid coz I heard there was a new form ,

please .. I do really appreciated your time every sec.
