Question on cover sheet for Affidavit Of Support packet

Hi

Good day!

I paid the bill today for AOS packet, I read that you dont have to wait for it to show as "Paid". I clicked on the "Payment Receipt" button then the "Print Document Cover sheet" button.

I need to know if it prints exactly what's on the cover sheet including the instructions, or do I just need the case info and the barcode on the bottom when I send it with package.

Thank you in advance. This is exactly what the cover sheet says below
