No DS-3025, do I need a new medical exam (I-693) with a civil surgeon?

Timeline

My wife had the obligatory medical exam abroad prior to coming here on a K1 visa, and we're applying for AOS now. The only thing she was given after her medical exam in Spain was an unopened package that ended up just having an x-ray in it. There is no other report. The only other thing we have is a payment receipt and a letter referring to her appointment with the doctor's stamp on it and her picture taped to it. Nothing about vaccinations. Do we need to make an appointment with a civil surgeon and have an entirely new medical exam? Do we need to schedule an appointment just for the vaccination supplement? Or do we even need to make an appointment at all?
