Does IO at interview test your grammar usage like verb form, structuring sentence etc..Wife has difficult of using them correctly.

The list of words for the writing portion is in the handbook and sentence is fairly simple. No, grammar etc.. Is not evaluated, just the ability to communicate effectively and understand questions and be able to respond to questions and follow a conversation with the IO

I saw a demo video on youtueb by Uscis. The English test is the IO will ask you to read a sentence and Let you write the sentence that he/she said. I'm not sure if ithis is accurate.

Again I'm linking the above. It is EXACTLY what USCIS requires. It gives the guidebooks and everything.

yes, you'll have to read a sentence and write one that he/she said, (often related to the sentence you just read). There's a tip sheet that has most frequently used words here http://www.uscis.gov/USCIS/Office%20of%20Citizenship/Citizenship%20Resource%20Center%20Site/Publications/PDFs/M-715_writing.pdf . It's usually pretty simple but some IOs do ask you to write pretty long complicated one. you get 3 tries. I think.

btw, you don't have to speak perfect English, as long as you can communicate with IO, that'll be fine.

that is what I thought. My wife says "I born in Vietnam" instead "I was born in Vietnam" any many many questions like that. I don't know if IO accepts her answers.

Basic understanding of English is required. You will be asked to go through the interview application in English, the civics test, and the read/write sentence piece. Based on recent experience, you do not need to be 100% accurate in grammar, but do need to ensure the meaning of the sentence is not affected.
