MEDICAL ON TUesday! Help with DS-3025 or copy of vaccinations?

Hi!,

Calling on all Irish peeps that went through a medical with Dr. Gleeson.

Does he issue a certified copy of the DS-3025 or a certified copy of the vaccinations to help with the adjustment of status process as stated in this paragraph in the K1 intstructions?

"NOTE: Tip Regarding Vaccination Records

The K-1/2 beneficiary will be required by the embassy to have a "medical" done prior to the visa interview. Part of this interview will require that previous vaccinations are documented and potentially additional required vaccinations performed. Once this is complete the doctor will note this on a DS-3025 form that will be sent to the embassy. During the medical appointment request an official copy of the vaccination records from the physician (or a copy of the completed DS-3025 if they will give it to you) as this will be helpful to have on hand in the future (specifically it can be helpful when filing for Adjustment of Status if your DS-3025 form is not available). "

I did my medical for the CR 1 visa with Dr.Gleeson. So I don't know exactly if they fill in this particular form. But what I can say is the staff at his office are very professional and seem to be on top of the whole visa medical aspect. I didn't see what forms they put in my medical envelope. But I did get back a personal copy of whatever vaccinations he gave me on the day. I didn't bother bringing any record of my previous vaccinations and this wasn't a problem.

I would call the office in advance and check, they are super helpful.
