Hello, just a couple of questions. I have looked around the forums but cannot find definite answers for these.

With part 2 - the issuance of my passport, do I put London or IPS U.K?

With part 30 - does a I-94 visa waiver count as being issued a U.S visa?( I'm assuming it doesn't, but just wnated to double check). I've obviously gone over to visit Jeff in the U.S but no had anything issued.

I would put London because it it the main location of the IPS. This being a worldwide form, some passports may list a city if issuance. My US passport does. I dont think there is going to be a problem with IPS UK either. One of our members researched it with IPS and they said that would be the proper thing for place of issue. The only problem I have with that is-- that was a British answer and this is an American form. I doubt IPS UK would be as familiar to an American as London since it says "city". Members have listed all kinds of answers before and none of them were rejected, so just pick one as it doesn't seem to be critical.

Visa waiver means a visa was not required or issued. Need for a visa was waived. You are correct.
