I'm in the US on a K1 Visa and I need help before applying for the AOS.

I realize there's a medical information I have to submit with the form.

When I did my medical in Brazil I didn't get a copy of it.

That was less than a year ago.

What exactly do I need to do now?

Should I see a doctor first?

Will I take the vaccination again?

I just want to make sure I'll send them everything, no matter what it takes.

Hi- Are you still in Brazil? If so, try to get a copy of your vaccination worksheet(DS 3025?) from your doctor and if not, still try to contact him/her and he/she might be able to fax you one. You'll need this and take it to a civil surgeon in the US (in the USCIS list of doctors) to have that info transferred onto the I693 vaccination supplement form. As you might see from other posts, many people have problems finding a doctor that will do just that without telling you you need the whole medical, so try and research a little and you might be lucky in your area.

If you can't get the form from your doctor in Brazil, you might need to have the vaccinations again...

November 3, 2006-CRIS email- Transfer to CSC! :)

November 7, 2006- I485 touched, CSC received case :)

November 8, 2006- Biometrics appointment- San Antonio-done :)

November 9, 2006- AOS touched

November 27, 2006- CRIS email- Green Card Ordered!!!

November 30, 2006- CRIS email-Welcome notice sent

December 1, 2006-AOS touched

December 2, 2006- Green Card Received :) :) :)

Hi rlt,

I'm from france, and in the same situation as you.

i didn't get a copy of the medical exam i had in france, so now it's really painful to get a doc that will do the paper for me.

i went to an appointment yesterday, they told me i had to do the physical again (even when i showed them the AOS form saying the K1 only need vaccination supplement).

anyway, i did it, but then they didn't make me the vaccination paper cause my vaccination record are in french.

so i have to wait next week, another doc will be there and maybe he will do it for me, or i may have to get my record translated, or get the vaccination again :o
