It appears the online form for DS-156 found at evisaforms(dot)state(dot)gov website is not working.

I have tried multiple browsers, all have the Acrobat Plugin v11+, and all give me an http 500 error, something like:

"An error occurred on the server when processing the URL. Please contact the system administrator." (Firefox, Chrome)

or

"This error (HTTP 500 Internal Server Error) means that the website you are visiting had a server problem which prevented the webpage from displaying." (IE)

Can anyone corroborate this please?

If you can get it working on your computer, please let me know.

If not, help me brainstorm ways around this.

stat.gov has the fillable PDF at http://www.state.gov/documents/organization/108128.pdf, but you would not get the generated barcode from the online form.

To specify, I don't get any errors and can fill it out, but when I hit continue, which is supposed to generate the form, I get the errors I stated earlier

THIS IS CRAZY! MY FIANCE'S INTERVIEW IS ON TUESDAY AND SHE CAN'T COMPLETE THIS FORM ONLINE!

We don't know what to do. I made phone calls all day, after a few calls I was told to call the first person that I called. WE dont know if we should cancel her interview or go just with a printed ds-156 with no barcode. any advice to give?

I am also having problems in submitting my form online! My interview will be on August 8. That is not my only problem, I was so excited before I received the approved letter from the consulate and filled up DS-160. Then I realized I made a mistake. I need DS-156! I've been trying to fill out the form on online, however, every time I submit my completed form, I always get error. i've been trying for 3 days now and it's not working still. I updated versions of my internet explorer, adobe and stuff. I went to several internet cafes, hoping that it was just my desktop, laptop, and office computer. I thought it was just me, then I bumped into this forum. So, it's not only me who is experiencing this problem.

My form does also not process.

Does anybody know what is going on? So frustrated right now.

K1 Visa

NOA2 - 13.04.13

Packet 3 received - 07.05.13

Packet 3 sent - 26.06.13

Packet 3 logged on CEAC - 09.07.13

Medical - 05.07.13

Medical results received at embassy - 10.07.13

DS2001 received at embassy 12.07.13

Last status update on CEAC 10.07.13 - Ready

Packet 4 received - 07.08.13

Interview - 15.08.13 - Approved.

AOS

AOS/EAD/AP filed 16.11.13

USCIS received applications 18.11.13

Biometrics appointment 13.12.13

Waiting..

What we are getting is an "HTTP 500 error" This means that there is a programing problem or server problem on their side of things. There is nothing we can do about it. If you are getting the "your computer doesn't meet the minimum requirements" popup then you need to update your browser and your adobe acrobat.

I got an email from the US Consulate in Canada and they stated the form is working and to please try again. I tried again this morning and had no luck.

I also tried entering in the required information with out punctuation and that didn't work.

I guess you just have to keep trying.

They do state that the barcode helps them process the document faster, but if it doesnt work, I suggest getting the fillable pdf at http://www.state.gov/documents/organization/108128.pdf and then explaining the situation to them, maybe even print out a screenshot. You wont have the barcode, but the form should still be valid
