This topic has been covered before. I just want to share our recent experience. Since my fiancée had an annulment the Filipino pre-screener took the original annulment decree and finality which was registered at the city hall. The Filipino pre-screener advised us the embassy did not need the original they would accept a copy. Since we forgot the copies at home we told the Filipino pre-screener the embassy was more than welcome to keep the originals. We also provided the annotated Marriage Contract obtained from the NSO. The embassy did not ask for the Decision. The embassy did ask for the CENOMAR . Yes the embassy knew my fiancée was annulled and asked for her CENOMAR....

Pop quiz... What did we give the embassy????

Answer: CRS Form 5. Advisory on Marriage. (Filipino pre-screener said "thank you")

May i ask, what is that CRS Form 5. Advisory on Marriage?

Its the CENOMAR for married people.

To get this, you need to fill-out an application for CENOMAR at NSO or you can order it via phone at 737-1111.

Married people can get a CENOMAR too. They will get the CRS Form 5 - Advisory on Marriages, which will show their past and current marriage.
