I-134 form help please!

I am filling out the for i-134 and have a few questions. I work in a restaurant therefore on the form my manager will write my salary as $8 dollars an hour plus tips instead of a yearly salary i.e. $30,000. I will provide my pay stubs showing my claimed tips plus hourly wage and that will be more than the poverty minimum. Will this be sufficient or will they need a yearly salary written by my manager? It would be very hard by my manager and also a pain for them to calculate that number because I have not worked there for a year. Also second question would be is do I need to include my tax return? It is only required if you are self employed correct?? Thanks in advance!

Not all consulates require the most recent tax transcript with the I-134, but most do. Go ahead and include a copy. They are free from the IRS. If you have proof of how much you earn in tips, then how your manager worded it should be okay. Just make sure the documentation you present shows a clear picture of how much income you receive in totality.
