Having a maiden name and transition is no big deal, USCIS and the embassys see this all the time. After you get your GC with the name as you applied for it (your married name), you can change the social and dl etc. My wife still has her maiden on her passport. DL,GC, SSS all have married name, no issues ever traveling internationally or domestic. Your Dr. knows his job unfornately they got your DOB wrong. Fill out your AOS, name as you want it on the green card and list your AKA's. Good Luck

In Arizona its hot hot hot.

They got the name from my international passport. I will see her tomorrow and I pray she corrects it. Thank you all for your response

You would need to show a government issued id with the name you want. That is, your driver license or state id with the new name; which you should be able to get with the marriage cert at your local DMV or DPS

In my experience, they won't let you change it without showing your SSN AND marriage certificate.

smile4drill, I had actually commented previously saying that the I-485 form asks you for other names you go by. Hence why I didn't think it was that big a deal.
