Hi everyone, I really need your advice. Am trying to put in my AOS. I am in the state on F1 visa. Got married last December. Now we want to start the process but my husband does cannot meet the poverty guideline and we already have a co sponsors. My main question is that on my wedding certificate I changed my last name but on my medicals the surgeon mistakenly wrote my dads name as my last name, my question now is should I continue with that or the one on my medical.

January 28, 2014: Production of new Green Card ordered

February 3, 2014: New Green Card received; done with USCIS until fall of 2023*

December 18, 2023:  Filed I-90 to renew Green Card

December 21, 2023:  Production of new Green Card ordered - will be seeing USCIS again every 10 years for renewal

I would not. I would go back and ask the doc to fill out a new I-864 and explain him why. You'll have a huge mess with 5000 different names. As of the financial part, your husband files that, but can have co-sponsors (friends/family members) who can also be included. I hope this helps. Good luck!

Hey again:

The doctor should not get mad at you for this, but if he does that is his problem. If this is how he likes to live his life, fine! He is however, responsible for filling out a form correctly. Since you have changed your name already, it is his mistake now, so he'll have to do it whether he is upset, or not. I would take along the wedding certificate that shows your "new" name. Good luck!

Thank you for your reply. I will be going there tomorrow since am picking up the result then. My last question for now is that I misplaced my i-94 which I already applied for a replacement. The last notice I got was that it has been approved for replacement but the new card has nt been issued. What should I do when asked for my i-94 number? Thanks again.

So, what I hear from this forum about I-94 cards is that you need that. So, you should wait until you get the "replacement" card. I honestly don't know how strict they are on this issue when it comes to F1 students, but....I guess better safe than sorry. So, I would say wait until you have it hand, and then submit the package. Until then you can flip through the package, look through it again, make sure you have everything organized as it should. Plus, it may well be that you have to wait a couple more days for your new medical if the doctor is busy as then he won't sit down to rewrite it at that very moment. All the best!

I don't think it's going to be a huge deal, since you have to fill out "Other names you are known by" on the forms, but I agree that it should be corrected. I'm wondering how the doctor had the OP's maiden name in the first place.

Maybe the doctor had her maiden name through DL presented for identification purposes? I don't know. But I would correct it....I feel that is safer...but that's just me. o.O

I don't think it's going to be a huge deal, since you have to fill out "Other names you are known by" on the forms, but I agree that it should be corrected. I'm wondering how the doctor had the OP's maiden name in the first place.

Safer, yes, and I agree that she should get the doctor to correct it. My perspectve is from the fact that I could have applied for my LPR status with my married name even without changing it with SSN, I also recall that I had to include my maiden name on the 485 anyway.

My doctor did not change my name because I had no ID with my new name. They said that they can only use a name what is on ID.

But I filed AOS papers with my new name. They ask other names used on applications so it's not a problem.

To get new papers from the doctor takes time. They messed up my DOB and had to order new lab papers. It took 2 days + we had to drive back 1.5 h.

4/16/2013 NOA1 e-mails (x3) (routed to the USCIS National Benefits Center for processing)

5/14/2013 I485 Status changed to Testing and Interview. No updates on EAD
