Hi,

My fiancee is from Canada, and we're currently in the process of a K-1 visa, and I recently received the packet of information from the U.S. consulate in Montreal where she'll do the K-1 interview. I brought all of the documents to my parents house to get scanned to send to my fiancee when I realized that I'm missing one page in the documents checklist that she needs to fill out and mail back to the consulate with form DS-230. She can't mail in anything until she receives this page, and I've tried looking everywhere to find an electronic version online. I'm just not having any luck whatsoever.

Do any of you have an electronic version of the checklist (or know where I can obtain one) specifically from the U.S. consulate in Montreal? Any help will be greatly appreciated.

Are you talking about OF169? You can download that and other forms from other sites like Venezuela consulate website - they are all the same. Good Luck

That seems to be it (it has all of the same boxes to check); thanks for the help on nailing down the name of the form. The only one I can find online is from a consulate in Mexico, and it looks slightly different. Do you know if they have one for the montreal consulate? Do you know if it matters if looks slightly different and comes from a different consulate?
