Where can one obtain these forms ? My fiancee lives in Ormoc City . Who do I tell her to see about doing these forms ? She is 25 and I think we only need the advice affidavit , but we plan to do all 4 . I want her to be armed with everything but the kitchen sink . I heard that sometimes the CFO may ask for these . Does the office have these forms or will I need to create them for her ?

Did you just do that recently ? . A few days ago I saved the jpegs and made them PDF and enlarged them , but came out blurry . I retyped them today and already finished . Oh well it gave me something to do . I must say the site is AWESOME ! Thank you so much for doing it .

i apologize. i just updated it now. sorry for the inconvenience. :D

the JPEG that are on the sites are mostly print screens since embedding the document makes the loading to slow down... after i saw ur post, i've already decided to upload and embed the files so that it's easier to copy and print the actual files.

Then again the Philippines has been in my experience good for one and not the other .

Ohh. the messenger is under the "blog" tab. we also couldn't log in here on VJ with our original robb&ann account.idk y. lol. anyway,

for ages between 18-21, they were required/asked to bring a parental consent.

but if more than 21 y.o but LESS than 25 years old, a parental advice is what CFO will look for though not all were asked for this,

Your fiancee is 25 years old already so they might not ask for the advice form, the less than 25 years old is 1 day before the birthday and not while still at 25 years old..

but we don't really know what their flavor of the day is or definition of the required ages for the parental consent or advice form.lol. so it's better to be prepared.

the other Sworn statement if you will notice is sort of like letter of intent to marry and to travel to USA.. i got that knowing that the CFO also asked some applicants before to make a sworn statement sort of thing that they will travel to US and marry.. it was actually asked for those who are between 18-21.. that sworn statement i have is sort of like the application for legal capacity to marry but just made some changes at the bottom part into intent to marry and travel to USA and live with my fiance..
