I looked on the forums and found a sample for third party affidavit

I have a question on the part that says this for it:

1. I am making this affirmation in connection with the application of Konrad Alan Krakowiak for a visa for the United States of America. The contents of the affirmation are based on my own knowledge and are true.

can i just the visa part to "for an adjustment of status"?

if anyone has any ideas for sample's that worked for them let me know :)

Also is it okay to get the affidavit notarized the same day we get married? Obviously the affidavit will have information about our relationship before marriage.

08/14/2013: EAD Approved and went into card production (via electronic notification)

08/14/2013: AP approved (via electronic notification)

08/17/2013: Received Hard Copy of interview notice

08/20/2013: Received status update e-mail saying combo card is being mailed!

08/22/2013: Received Combo Card in the mail smile.png

09/11/2103: AOS Interview at 10:50am!

09/11/2013: APPROVED!! good.gif

and titling it?

Timeline

A letter doesn't need a title. They just put their name address and phone number in the corner as they would on a normal letter and then write their paragraphs below.

To end, its just the person ending a letter as normal and signing their name.
