Hello everyone.

I came to the US under a K1 visa in June of last year. For personal and financial reasons I am just now filing for my adjustment of status, and I came across a problem, or a few problems:

First, it has been over a year since I had my medical in my home country Portugal...do I need to have another one?

Also, I do not have a form I-693 or a vaccination worksheet with me. I know the civil surgeon in Portugal gave me the vaccination worksheet , i remember seeing him filling it out and it was complete, and i remember he placed it inside the sealed envelope he gave me. Uppon arrival in the US, i gave this envelope to the immigration officer but he never gave me anything back...I am assuming he kept my vaccination worksheet by accident?

I did however bring with me my vaccination record booklet from Portugal, where it states all the vaccines I have taken throughout my life, including the ones I had to take for the K1 visa.

So...how do I go about getting a new vaccination worksheet?

I was thinking about going to the local civil surgeon here and having another medical, and I hope he can look at my vaccination record booklet I brought with me and be able to transcribe it, but I am afraid since it isn't an american document that he will not accept it as being legitimate and I will have to get all the vaccines again. (I had to have 8 vaccines for the K1, and spent almost 2000 euros on them!!)

Any thoughts?

Print out 693 from USCIS. Take it to a civil surgeon you need a FULL medical done. Your medical is expired.

Bring your vaccination booklet they will copy the vaccinations over during your full medical.

Your I-129f was approved in 5 days from your NOA1 date.

Your interview took 67 days from your I-129F NOA1 date.

AOS was approved in 2 months and 8 days without interview.

ROC was approved in 3 months and 2 days without interview.

You have to have a new medical. When you see him, give him your vaccination card and he'll fill out the form.

If the beneficiary's vaccinations were COMPLETED as part of the visa process, IT IS NOT NECESSARY TO INCLUDE FORM I-693 IN THE AOS SUBMISSION. All your medical information is enclosed in the brown envelope which was handed in at the POE. The envelope is then forwarded on to USCIS where it will be consolidated with the I-485 submission, once received. If you received a copy of the Vaccination Documentation Worksheet (Form DS-3025) from your Panel Physician, you may include a copy of that form in your AOS submission in place of Form I-693.

So I guess that's pretty self-explanatory. I'm sorry I missed it before I made this post...

aid received the vaccination worksheet to get my green card but still I have not been able to vaccines and the letter says I have until February 3 to send the proof of vaccines, says the case may fall if not delivery time happens if I can not send time? urgent help.
