My wife does not have this (DS-3025) from her medical examination when she went for her exam back in March of 2006 at the Moscow medical office. How can she obtain this and is it necessary for the AOS interview. Any help would be appreciated as we filed the AOS paperwork without any I-693 or I-693A.

You will need to produce it for your AOS, so best to contact the consulate to see if they can send you a copy and then you can look for a civil surgeon. If you can't get the vaccination worksheet you may need to get all the vaccines again (yes you can get a test done and not receive shots for stuff that you have already received, but it may end up costing the same.) The USCIS has the DS-3025 - it's one of the documents that was in the sealed envelope that was turned ove to CBP on entry. So, the case officer who ultimately decides your application will have the form when the time comes.

Thing is, the people who give the preliminary inspection to your I-485 submission don't see the case file - they see only the I-485 submission alone, which in your case includes neither a copy of the DS-3025 nor an I-693 vaccination supplement. So, you may get an RFE for it.

She should NOT get all the vaccines again. That might not be safe. There are some tests that can determine what vaccines (if any) she needs to get.

Does she have any vaccination records from her home country? It doesn't have to be DS-3025, anything will do. Perhaps you could contact the doctor who did the medical and ask them to send a copy of the vaccination sheet? (if they still have it)
