I am the petitioner for a fiance on a K-1 visa. My fiance interview is June 4. I already filled out I-134 form and signed and dated it for him, however today i received another letter (dated a month ago) with the IMMIGRAND VISA CHECKLIST and the AFFIDAVIT OF SUPPORT part it has (FORM I-864), there is no mention anything about I-134. WHAT IS GOING ON HERE?!? Which is the correct form. Do i need to fill out both of them?

I believe that it can depend on the embassy -- it's their discretion how to evaluate the Affidavit of Support and what forms to require. You should probably contact/email the embassy directly to ask whether they'll accept the I-134, or wait for other recent Jamaica filers to chime in. Most embassies use the I-134 but I'm pretty sure I've heard of a few exceptions -- Jamaica might be one, or they might have made a mistake.

That is what i am hoping that they made a mistake because packet 3 mentioned the form I-134 and had a blank copy in it and now this packed that comes out of no where less than 2 weeks from interview says nothing of I-134 but mentions form I-864.

Also does the form I-134 need to be NOTARIZED or signed and dates is sufficient. I didnt see anywhere saying about notarizing the document but some people are posting that it needs to be.

Several years ago it used to have to be notarized. Now it does NOT need notary. Some lawyers and many older references still refer to the notarization, and it certainly never hurts to notarize it, but there was a reform in (2008? 2009? sometime) that changed the form and it does not need notarization these days.

Thanks, I guess i will just hope that the filled out I-134 is sufficient enough and hope someone from Jamaica would let me know if i need to send the I-864 form in or not at this time.

I have seen others from Jamaica state it is the I-134 for a K-1.

This is from the Kingston info page here on VJ> FOR K-1: Letter w/case number, 3pg instruction sheet (with all the info and forms you will need), Photo Specifications, DS-230, Instructions for ON-LINE Completion of DS-156, DS-157, DS-156K, Evidence Which May Be Presented To Meet The Public Charge Provisions Of The Law (DS-2000), I-134, Vaccination Requirements, Information Sheet for your Medical Examination (2 pages), Medical History Form.

You only need i-134 with a letter from your employer, W-2's for two years, plus 6 months of checkstubs for K-1. Yes you do need a copy of your birth certificate. It has nothing to do with your proof of citizenship.

The embassy emailed me that they only need I-134 form not the I-864. Also they want my original birth certificate (which is in another language) i will be translating that myself and notarize it. Also they never replyed to me if original or copy of the Certificate of Naturalization is needed as i asked them in the email. The notary refused to notarize or copy that document as it is against the law. So they must be crazy really to ask people to send the original of such important document over the mail. Smh

Am I reading this right...that my fiance will need a copy of my Birth Certificate for the interview? For the original petition I just copied all pages of my passport and that apparently was OK since they approved it. I'm not sure where mine is so I'm wondering if I should quick and order a copy? Thanks.
