I married my wife on April 15, she came here legally, and stayed legally the whole time I believe she was supposed to come back last October but before that she applied for her visa extension, what end up happening was that we got married and we applied for her green card(we did send a letter to withdraw from the extension and and sent the application for her green card before the end of her deadline)

My questions are:

Do I need to fill i-485 Supplement A?

I've been searching for an hour and found multiple answer about question 16 on the form i-765?

No, the I-765 fee is waived when the application is sent with the I-485 (or when your I-485 is pending).

Have you filed a I-130 petition also?

Yeah I have filed the I-130 before the end of her deadline and we already got the letter of notice from them, and case number, my wife has completed her medical exams and we got the sealed envelope, just tiding up the last questions to fill the i-485.

I've got another question when I sent the i-130 package, I sent to the Arizona USCIS, but now it tells me that the i-485 package has to be sent to Chicago? is that correct, but it sounds to me a little strange.

You're welcome. Be sure to check and re-check your forms, to re-read the instructions and be sure your package is complete. You want to minimize your chances of getting a RFE and so, delay the process. :)
