I tried it for so many times, one thing you need to do is to fill it up then print. No way to convert it in adobe. Ensure what you have entered are all correct entries.

i just had a problm with download manager , i think it's possible to save and register the data now on the form , on the contrary i had before just to fill it up then print ,

I dont know if they updated the form or i'm using a wrong form

Something is probably wrong with your copy of adobe reader. The form allows you to save data..and don't need a "pdf writer" or adobe acrobat. Download a stand-alone installer, without using adobe's download manager.
