I've been going through the forums for a few hours and I can only find old references and not so clear statements about the subject at hand.

I would like to get a clarification on how can one get, -if possible at all- a DS-3025 when undertaking the medical exam in Madrid at either "Centro de Radiodiagnóstico" or "Unidad Medica Angloamericana" so that we can use that later when filing for AoS, via transfer to I-693A or whatever.

I'm looking for recent visits to one of these clinics in Madrid of people who manage to get a copy of a properly filled DS-3025.

So far, what I have is some of my old vaccine records, in the form of little booklets that they used when I was a kid getting vaccine shots. I'm attaching a picture of one of them. I have an appointment in a few days with my local doctor to try and get a copy of my medical history, and see if any of those vaccine records made it there.

Having done some research on AoS, I think this can save a lot of headaches to many.

I know one thing for sure - they don't just give it to you. I think you're ahead of the game if you know to ask for it. My husband was unfamiliar with the process, so he didn't know that he even needed it (did the physical in December 2012).

I'm going through AOS right now, and it will definitely help if you get this form. If needed, you can even point to the line at the bottom of the form that says "Give copy to applicant." Good luck!

Back in September I had my medical checkup done at Unidad Médica Angloamericana. I asked for this form, I made clear I would pay for it, they said vaccinations are not mandatory for a K1 and thus, they won't issue it.

However, I made sure I'm up to date with all the vaccinations for AOS before leaving Spain. And I managed to convince my doctor to enter all the old vaccination records from the Cartilla de Sanidad into the electronic system they now use. This way, I was able to get an official document that shows my complete vaccs history before my trip. I then gave a copy of both this certificate and all the pages in the Cartilla to the civil surgeon here in order get the vaccs supplement needed for AOS.



Hey Toni! If you can keep us posted (or at least me) on what you take to the medical in Madrid that would be great. My fiance and I are also confused as to what he needs to take and do for that. Thanks so much and good luck!

Thanks Mrs.Him, your insight always comes handy. I got back from my doctor with a sheet that says "I have all required vaccines" and a blood test showing I don't need Hep A & B shots, but that's it. I'm considering going back and asking for a full list of the vaccines, as you did, since it seems impossible to get it from the clinics.

I called both clinics today to set up an appointment and ask about the form, and I got quite an unrelated and unexpected surprise. Centro de Radiodiagnostico (915474527) said they don't do medical exams for visa purposes anymore. Of course they're still listed in Packet 3, but 10 year old DS-* forms are also included in Packet 3 so I should have expected that.

Then I called Unidad Medica and the friendly lady that made the booking understood my enquiry and added a note to my appointment saying it's for a "fiance visa with a vaccination record". She did say however that the doctor will have the last say on whether I can get the vaccines transferred to the DS-3025 or not. So I'm expecting a very polite "no" from the doctor, we'll see.

I will keep you all posted with the outcome.

I will keep you all posted with the outcome.

As promised, here is what happened in the medical. Look for my consulate review if you want to learn about the whole visit to Madrid (not published yet at the time of writing this).

As mentioned earlier, I had left a note with my appointment at the clinica Angloamericana saying that I wanted my vaccine record recorded by them. When I got there they acknowledged the note, and after the doctor's check up I was sent to the infirmary, where the vaccine thing would be taken care of.

Well, it was impossible. She said everything we've heard before in these forums, the first and most important for them: "we can't provide DS-3025 to K-1s because we don't have that form, it's the embassy's property". There was a lot more chatting about AOS and whatnot, but in the end it came down to that single reason, why they couldn't do it. They even said other visas couldn't get that either, because the DS-3025 goes straight to the embassy. "The person getting the visa can get hold of a copy of that form, it's written at the bottom of the form", I said. They had no idea apparently.

After I suggested the system was not working in that aspect because K-1s should be able to get hold of that form they said they couldn't do anything else. Of course they can, but they don't want to. They're very happy getting 165 EUR from each of us for very, very little work. And they don't plan on changing a formula that gives them a lot of benefit. So after giving in and accepting they won't even try to change it for the future, I asked: "Could I simply have all my vaccine records (I had the original childhood vaccine records and some other documents acknowledging I have shots for so and so) recorded in a piece of paper with the clinica's stamp?". "No, we can't do that for K-1s".

Whatever. They're nice people and all, but they won't go the extra mile to help you. Not even an extra 10 feet.

Now I will try to get the list of shots in one single piece of paper with some official stamps from my local doctor. I'll take that and a copy of my childhood vaccine records, as Mrs. Him suggested previously, and hopefully that'll do.
