I 693 and confusion about name change

Hi,

I am changing my name on all the AOS documents ( I 130, I 485, I 864, I 765, I 131) from my maiden name. I did not change my name officially when we got married like on the marriage license or marriage certificate.

And I am using immigration services as an official way to change my name.

I ran into problem when filing out I 693 because the doctor said he can only give reports to person with a valid ID. On my passport and all the valid ID's I still have my maiden name. So I am now confused as how to proceed with this.

Is it okay to have my maiden name on the I 693 and changed name on all other forms? Does anybody have this problem or faced when filing for AOS?

Will they accept I 693 with my maiden name and all other forms with changed name?

Would it cause confusion?

In short, you can show them your ID with your maiden name and your marriage certificate and this will serve as proof that your new name is legal and can be used on the I-693.

You may need to direct the civil surgeon's office to this memorandum if they are being difficult about it.

You can use maiden on I-693 and put married name in parentheses (). That's what i did with no issues.
