changing last names on ROC

My wife came in on a K1 visa and literally the next day we got married and got her a SSN.

We opted to not change her name until we did the ROC because I had read elsewhere it would be free.

Am I correct to put Name - New Last Name under part 1a-c

And then list the name on her GC under Other Names 2a-c?

and one final question. I don't have a middle name. Should I leave that area blank or put N/A?

also, it would be great to get a new i-751 example since the one the forum has is old

Jun 20, 2011 - RFE hard copy (due in 87 days)

Jun 21, 2011 - RFE mailed out

Jun 22, 2011 - Biometrics in Boston

Jun 29, 2011 - 'touched'

Jun 30, 2011 - Travel Doc approved

Jul 07, 2011 - Work authorization approved

Jul 11, 2011 - Work Authorization received

Jul 13, 2011 - Received notice for interview Aug 15

Aug 15, 2011 - Approved! FINALLY DONE! (for now)

Oct 03, 2011 - Finally got the damn GC, except they put issue date 2007, expiration 2009 instead of issue 2011 expiration 2013. Rather than reissue the damn thing, they sent an i-90 form to complete and request a new one! Why?

Just a heads up - it would have been free to just do it in the beginning with the AOS when you filed using her married name.

Your I-129f was approved in 5 days from your NOA1 date.

Your interview took 67 days from your I-129F NOA1 date.

AOS was approved in 2 months and 8 days without interview.

ROC was approved in 3 months and 2 days without interview.

Just a heads up - it would have been free to just do it in the beginning with the AOS when you filed using her married name.

wasn't an option, it would have created a financial headache for the both of us......
