Affidavit Confirming Marital Relationship

In the instructions for the I-130, it states that I can include:

"Affidavits sworn to or affirmed by third parties having personal knowledge of the bona fides of the marital relationship. Each affidavit must contain the full name and address, date and place of birth of the person making the affidavit, his or her personal relationship to the petitioner of the beneficiary, if any, and complete information and details explaining how the person acquired his or her knowledge of your marriage)."

I'd like to include this in my petition, but I'm terrible at writing such things. Can someone provide me or point me to an example of such an affidavit? I will be including affidavits from both of my parents.
