Hello Everyone,

I'm currently filling out all the paperwork for my AOS.

I entered the USA on april 22, 2013 with a K1 visa and married on may 03,2013 to the love of my life.

So far I filled out all the neccasary forms I-485, G-325a, I-864, I-765 & I-131. (do I have them all?!?)

I haven't completed all my vaccinations yet. I had 3 shots in Aruba and tomorrow I have an appointment at the Immunization dept for 2 more shots. I still have to get 3 more; 1 in june and the last 2 in october.

I have read on here that I have to fill out form I-693 since I haven't complete the vaccinations.

My question is: Do i fill it out my self? Or can I have my civil surgeon in Aruba complete it? Or can I send a copy of my International certificate of vaccination with the appointments for the rest of the vaccination that I will get from the Immunization dept tomorrow?

Can I ask my civil surgeon in Aruba for a copy of the medical and add it to my package of I.485?

I'm so confused cause they told me I only have to go get the rest of my vaccinations and just add a copy of the certificate. But apparently that's not the case.

Any advice, help or tips will ben highly appreciated. we really want to send out the package by friday and this is the only info we still need.

You will have to make a new appointment with a civil surgeon in the USA, and bring your medical info from the last medical exam you had. The USCIS website has a list of appointed civil surgeons in the US. You cannot use a doctor from outside the US for this one.

Thank you for you reply. But I found out all I need is a copy of my DS-3025(vaccination form). Beside I didn't get any info on my medical, not sure if you deed. In Aruba we don't get it. I contacted my medical doctor in Aruba and he send me a copy of the DS-3025. Actually we are suppose to get a copy of this form at the medical. It's even written on the form. With this copy I added a copy of the vaccination I had since I got here and my next appointment. Did u finish all the required vaccinations already?

And an other question were you able to receive your visa on the same dat at the embassy like I did?
