Filling out the I-485 form. Part 2 which box to check?

Background. Husband came here to USA on a B1-B2 visa March 2012. This is his 2nd trip here, but overstayed this time. First time he returned home timely........ and later was issued the 2nd visa with no problems........... but now on 2nd trip, he is out of status for a bit over 6 months.

We married few months ago.......

I'm no sure which box we need to check that applies to our situation.....

Thank you for any help on this matter.

Filing the I-485 form, the I-130 form, and theI-765 form all together.

Do we have to send in the Medical form ( forget the form number) with the 3 above forms also? I think so, but do we go to our family doctor for exam and to complete form?

Only a designated civil surgeon can complete an I-693 medical examination form.

Day 376 (1/14/19) Interview

Day 385 (1/23/19) Denied

Day 400 (2/7/19) Denial revoked; N-400 approved; oath ceremony set for 2/14/19

Day 407 (2/14/19) Oath ceremony in Dallas, TX
