Sample Affidavit for ROC

Hello, I am getting ready to submit my ROC packet in 2 weeks time. Does anyone on here have a new/updated sample Affidavit ? I would highly appreciate it. Thank you very much.

An affidavit you copy from the internet will be bad and useless evidence.

An affidavit is something personal that someone who knows you writes about you using their own words. It is not a thing taken from the internet.

I understand it has to be your own words. I was looking for a Body (how to address them, information like drives license, social security, birth certificate copies) that needs to be included in the letter. I am getting three affidavits therefore i need to ask my three witnesses to also type the required information. Hence, I was asking for a sample so I can inform my family in advance of all the information they need to include in the letter when typing it.

look at the vjexamples affidavit you can change it so that it's not the same.make it a little different in words. I have known john doe and ann for xxxx amount of years we visit each other often we go to movies togather and each others house we are good friends john doe and ann is a loving happy couple always doing things togather thay are so happy a true marriage,my namexxxxxx,my addressxxxxx.my phone numberxxxx, date, sign in front of the notary,
