Who should sign and the date the Declaration of how we met in person, the US Citizen or the Alien Fiance??

Nobody needs to sign it. It is simply an attached page to your application due to the limited space on the application itself. You sign the application.

If I'm not mistaken, the petitioner must sign and date the supplemental additional page.

But if the declaration fits in the space provided on the I-129F, no extra page is needed.

Aww okay, well I didn't sign mine. I hope I don't get an RFE for that.
