Hello everyone I am going to be sending in my husbands paerwork for his adjustment od status.

My only question is being that he just had his full medical in the Dominican Republic do I have to have a Civil surgeon here sign the vaccination sheet on the I- 693 and does he need it or do I just send in a copy that they gave him in the Dominican Republic? this was in January that he had his Medical and immunization shots.

please anyone thank You for your help.

Was it marked off as complete? If not you may get an RFE for a completed one from a civil surgeon. I say may get an RFE because some get away with a record as incomplete to. It appears to depend on how picky the adjudicator who gets your file is. We didn't want to take the chance with my wife's and we found a reasonably priced civil surgeon in our area.

medical yes and he had vaccinations given to him.

The vaccination sheet must be checked off as all series complete and signed by the physician. If vaccinations are missing or marked incomplete, then you may get an RFE if you don't see a civil surgeon and get him to sign off on it.

medical yes and he had vaccinations given to him.

If he received all of his vaccinations at the time of his medical, then no you do not need to see a civil surgeon again. The medical is good for one year. They should have given him a form at the medical D-3025 which is what you are suppose to turn in with the AOS packet. If he does not have it you may be able to contact he clinic in the DR and see if you can get another one.

Yes I do have the copy of the D3025. and Actually the CD they gave him as well that says not to open that they never took when he turned in his medical packet. We still have not opened it just in case. it was attached to the D3025 copy should I send in with the AOS packet as well.

it seems so simple and complicated at the same time.

I don't think they take CD and things of that nature. I would call USCIS and ask about that. All I have heard of people sending was the D3025.

I don't think they take CD and things of that nature. I would call USCIS and ask about that. All I have heard of people sending was the D3025.

I'm pretty sure my wife has that CD saved somewhere, and we're through the AOS process.

Me: Irish/ Swiss citizen, and now naturalised US citizen. Husband: USC; twin babies born Feb 08 in Ireland and a daughter in Feb 2010 in Arkansas who are all joint Irish/ USC. Did DCF (IR1) in 6 weeks via the Dublin, Ireland embassy and now living in Arkansas.

Thank you guys once again. I love this site for all the great information and support from everyone.

It's a real comfort and reaffirmation knowing you have somewhere to turn to in this long but in the end worth while process.

The CD of your x-ray is yours to keep.

It is not as simple as whether or not the form was marked 'complete' or not. It is not common for the form to be marked 'complete' since you could have too short of a time between vaccines, and many of them are not age appropriate. If you had all the required vaccinations at the time of the medical and they were marked off on a properly filled out DS-3025 that states 'incomplete and may be eligible for blanket waiver', then you can still skip the civil surgeon and just send a copy of your DS-3025.
