Here are the I-485 instructions: http://www.uscis.gov/files/form/i-485instr.pdf

You don't need another medical and you don't need any more vaccinations. You simply submit your DS-3025 with your AOS packet.

However, some people here on VJ have reported getting an RFE for not submitting an I-693. If you want to play it safe, have a civil surgeon transcribe your DS-3025 onto an I-693 and submit it with your AOS packet. Chinook had her DS-3025 transcribed onto an I-693 by a civil surgeon. It cost about $35 and took about 15 minutes in the civil surgeon's office. The difficult part was finding a civil surgeon who understood the process. Some civil surgeons wanted Chinook to have another medical or more vaccinations.

Your right! I recently did an AOS for my wife and daughter that arrived here on K1, K2 visa. I did not submit I693 or got them any medical exam. I only included a copy of their immunization records with the AOS packet and we had no issue with it.

What I did is I went on the USCIS website and enter my zip code to find civil surgeon in my area. I called many civil surgeons but they all wanted me to do medical exam. Well..... I've already did medical exam for K-1 visa in my country, and it is less than a year. I keep calling different civil surgeons till I got the one who understood the process of adjustment of status from a K-1 visa, so I went to that one and the civil surgeon took all my vaccination records, she told me I've had all the required vaccines so that means I have a complete vaccinations, she only transferred it to form I-693.

Hi everyone! I had my medical exam at St.Luke's last April 22-23, 2013. I had 4 shots of vaccines on the second day: HepaB, TDap, MMR and Varicella. But I'm confused about their follow-ups/second doses? Can I just have them here in our province in Tarlac by our private doctor, if yes, will it be accepted by the USCIS? or I need to go back to St.Luke's for it, and if so, does it have an additional charge? I'm thinking that its better to have my follow-up doses than having it in U.S. (much cheaper I think).good.gif

Anyone who have an idea? Thank you. smile.pngsmile.pngsmile.png

i got my second shot of varicella here at us.. we asked the civil surgeon to transcribed my ds 3025 into I-693.. then he told me that i still have varicella shot left so to let him do what we wish him to do so.. we paid him 109 for everything.. its different of what they said to me on the phone they said we only need to pay 25 dollars instead of to pay 215 for my whole exam.. i better suggest to do it here... it cost that much or much cheaper if u got a right and kind civil surgeon.. lol.. just make sure the civil surgeon u've got knows about the process...

You aren't required to complete the vaccination series to become medically cleared. Because the follow-ups doses need to be done several months apart, there is a blanket waiver for the follow-up doses, as long as the first shot is given to you at the exam.
