if u have oustanding vaccinations canu get them at regular doctor. an then have the civil surgeon fill out he vaccination supplemental form? ( i recently did my medical for my k3 in september 2006)

Yes, just ask whoever gives you the vaccination for a record and give a copy of it the civil surgeon.

Maybe you can, but keep in mind that some civil surgeons might refuse to fill out the supplement, because a civil surgeon is supposed to determine what vaccinations you need to get (if any), and make sure you get all of them.

Only if the Civil Surgeon is an ###.

If one provides industry-standard proof to a physician of vaccines, there should be no problem with filing out a State Department form.

Rebecca, does this mean it's ok to get the vaccinations anywhere from anyone as long as they provide sufficient, industry-standard proof. (Is there such a thing? Just a form with the doctor's name and vaccination list?)

The local civil surgeon where I live quoted $250 for vaccinations but I know we could go elsewhere to a clinic and pay much less. I wonder if the CS generally charges to transfer the info to the state dept form. It never even occured to me to ask him that.

Doctors usually keep some sort of form for vaccines. Like the one you had as a kid.

I wouldn't hesitate to even go to the health department and get them for free as long as they could give me a viable vaccine record.

I simply wouldn't go to a civil surgeon who insisted on administering the vaccines personally. That's medical shystering.

The CDC website has a 100 plus page guide that civil surgeons are supposed to follow, you might want to look in the guide there and see what it says.

Valid vaccination record from elsewhere should be fine, if it isn't find another civil surgeon.

No, he didn't.

I don't think that they are allowed to charge extra for transeferring information onto the form, just a fee for vaccinating someone if it is needed. I checked the CDC guidelines and they are supposed to review any vaccination records submitted as part of filling out the supplement form.

We paid $35. I think most of them charge a small nominal fee. At least that's what I've always read on VJ.

Here is how mine went.

I went to the civil surgeon 1st and got the full medical exam and then he told me the shots I needed then he offered to give them to me but I told him I wanted my family doctor to do it. So I got the necessary blood work from my doctor and it also turned out I was immune to all the shots he said I needed. I found all this out because my family doctor actually tested my blood to see if I really needed the shots and all I had to take to the civil suregeon was the sheet from the lab with all the results saying I was immune to all the things he wanted me to get shots for.

We did not get Marlene's additional vaccinations from the civil surgeon. We got them elsewhere and just took the form from the medical exam in Manila and the record of the additional vaccinations to the civil surgeon. They charged us $25 to complete the vaccination supplement.

They just want money, any way possible. Of course, they will get the most if they give you a full exam, the shots, and fill out the paper. But they will still get a "paperwork fee" from you for just writing down the info.

Chances are very high you will have to pay that fee, even if you have all of your shots.

Another thing to keep in mind, is that insurance will not cover many of the vaccinations because they were supposed to have been administered during childhood.
