Passport in Cebu

Hello. My fiancée went today at the former office of DFA (not the one at pacific mall.) she had all requirements except for school record / voters affidavit. She talked with this lady at the table and paid up front 500P and left with this ladys number to set up an appointment for Friday once she comes with either a shool record/ votes affidavit. The lady said for rush order 2000 all in all. I guess I missed that last requirement it wasn't really listed in any guides. Does this sound legitimate ?

edit: there was no line & its a fixer, they're expensive so she'll try to get the 500p back and ill just set up an appointment online myself, what im asking is, the 4th req school rec/voters affidavit required?

LIST OF ACCEPTABLE IDS (At least 1 of the following):

• Government-issued picture IDs such as the following:
  • Digitized SSS ID
  • Driver’s License
  • GSIS E-card
  • PRC ID
  • IBP ID
  • OWWA ID
  • Digitized BIR ID
  • Senior Citizen’s ID
• Other acceptable picture IDs such as the following:
  • Old College ID
  • Alumni ID
  • Old Employment IDs

LIST OF SUPPORTING DOCUMENTS (At least 3 of the following):

• Voter’s ID
• NSO Marriage Contract
• Land Title
• Seaman’s Book
• Elementary or High School Form 137 or Transcript of Records with readable dry seal
• Government Service Record
• NBI Clearance
• Police Clearance
• Barangay Clearance
• Old documents issued at least one year prior to date of application that show correct name, date and place of birth, picture and signature of applicant such as the following:
  • NSO Birth Certificate of child / children
  • SSS-E1 Form or Microfilmed Copy of SSS-E1 Form
  • Income Tax Return
  • Voter’s Certification, List of Voters and Voter’s Registration Record (for newly issued VRR, please attach receipt)
  • School Yearbook
  • Community Tax Certificate

Aside from her NSO birth certificate, she should provide 1 acceptable ID or at least 3 supporting documents.

I get my passport with less requirements. NSO b-cert and SSS only and pay P950 got it in 3 months

I don't think you need to pay that much for a passport. Your fiancee can just bring 1 valid id, her NSO birth certificate and perhaps NBI clearance. Normal processing costs 950 php, I guess. You can also opt for express processing. That would cost 1,200 php and your fiancee can get her passport after 25 days .

Hello. Online appointment is only for Manila, to get an appointment in cebu does she just go to DFA in pacific mall?

No online appt in CEbu. She has to be there early in Pacific Mall in Mandaue.. I think-not sure though, if you only have 1 govt issued ID, that's when they will ask for TOR or school records. My brother was also required to present TOR when he applied for a passport for the first time last year and he was not even asked for an NBI clearance. My bro has only 1 ID w/c is his drivers license..
