UK Vaccination Records

Got my DTP done this morning in fact, and the lovely nurse just printed my full vaccination record out there and then (for free) after she added in today's DTP to the list. Another form down!

My question is though - I am moving house next month, so the address that's at the top of the vaccination records will be 'old' by the time I come round to my medical. Will I need to get another print out once I am in my new address?! (Hope not...)

USCIS (239 days):

04.18.2013 - NOA1

12.13.2013 - NOA2 via email; I-130 approved. Shipped to NVC

NVC (274 days):

12.30.2013 - Arrived at NVC

01.27.2014 - Received case no. & IIN

02.07.2014 - Completed & submitted DS-261 form online

02.13.2014 - Paid AOS bill

02.18.2014 - AOS showing as 'paid'

03.18.2014 - Invoiced for IV bill (finally!)

03.19.2014 - Made payment for IV bill

03.21.2014 - IV bill showing PAID

03.24.2014 - Completed DS-260 online

03.29.2014 - Mailed IV Packet

04.26.2014 - IV packet all approved, false checklist from NVC for AOS packet

05.07.2014 - AOS packet sent

06.12.2014 - AOS packet scanned in (took them THAT LONG to do it)

07.30.2014 - Sent re-filled AOS form, as per checklist instructions

I don't know if having your old address on your vaccination print out would be an issue or not? My vaccination print out had my maiden name on it instead of my married name and they didn't question it. I wouldnt think it would be a problem. Perhaps you could give them a bell?

The address doesn't matter as long as it has your name and some indication that it came from a doctor office---letterhead, stamped, nurse sign, doctor sign, office name prints at top....

It could be an old shot record form your Mum had in a drawer from 3 addresses ago, plus a new one adding shots from last week, plus a flu shot paper from Tesco. Knightsbridge will combine them onto the one DS-3025 form.

One last thing on vaccinations .....

I expect to have my medical sometime over the summer (i.e non flu season) but will probably be travelling after October 1st which I reckon is in the new flu season. Is this likely to present any sort of issue when I AOS ? I'll have the official record for my other other vaccinations as part of the medical exam but won't have any "official" record of flu vaccination. I'm aware that I can get a flu jab easily (and cheaply) enough in the US but was just checking there were no gotchas.

I just got mine printed out by the receptionist. Its entiteld "Full patient print" and has a long list of all my vaccinations since birth, at the bottom it has my name, patient ke, NHS number and a "code" aswell as my DOB, but it has no doctors address or signature, will this be ok?

My mum managed to find my baby health book with a page of dated immunisations and tucked inside it are two forms from when I was at secondary school; consent forms my parents signed and at the bottom the record of the injections in question (BSG and booster for Tetanus/Diphtheria and Polio). From the reading I have done on here, I'm led to believe these documents serve just as well as a print out from the doctors and the info therein can be brought together on the form Knightsbridge fill in and give me a copy of...?

Due to my going to visit my fiance for several weeks, soon, I'm not going to be back at my GP until early August to sort this stuff out, but I think rather than hand over their charge of £20 I may just go armed with baby book etc to my medical, whenever it ends up being... smile.png

Do you know that for US immigration Tdap or DTP is Tetanus/Diptheria/Pertussis?

P is not for Polio.

And you need something in the Tdap, DTP, Td range no longer than 10 years ago, so check your dates.

Do you know that for US immigration Tdap or DTP is Tetanus/Diptheria/Pertussis?

P is not for Polio.

And you need something in the Tdap, DTP, Td range no longer than 10 years ago, so check your dates.

Well just got back from my GP (practice nurse). She gave me the appropriate tdap but wouldn't give me an MMR before she'd checked my paper records which (to my surprise) they actually do have in the basement. So she's going to call me. Now I just need to double check if there's a time expiry on MMR because if I did have it , it was a long time ago being born in '57.

And there was no charge - I will sooo miss the NHS.

It helped that the nurse may well be moving to Seattle with her partner.

I am under the impression that there is no expiry on the MMR, you just have to of had two lots of it (one would be a booster...)
