Medical record for medical examination

Timeline

I have a copy of my medical record on the headed doctor`s surgery paper. It is not officially stamped or signed. Is this official enough to take along to my medical examination in London?

Also, will i need any other shots? As only my senior school shots are actually listed on the record.

It had the following listed:

28/06/1995 BCG Stage: 0 Given Due 16/08/1995

22/11/1994 RUBELLA Stage: 1 Given Due

22/11/1994 MEASLES Stage: 1 Given Due

10/06/1992 RUBELLA Stage: 1 Given Routine Measure Due 10/10/1994

16/06/1991 POLIO Stage: B Given Routine Measure Due 16/06/2001 taken from old notes -course copmpleted upto this date

16/06/1991 PERTUSSIS Stage: B Given Routine Measure Due taken from old notes - course completed upto this date

16/06/1991 TETANUS Stage: B Given Routine Measure Due 16/06/2001 taken from old notes - course completed upto this date

16/06/1991 DIPTHERIA Stage: B Given Routine Measure Due 16/06/2001 taken from old notes - course completed up to this date

The headed paper should be fine.

I don't see mumps, so probably a MMR.

Td because it has to be in the last 10 years. You've had one round of tetanus, diphtheria, pertussis so the Td booster instead of the Tdap.

No varicella if you've had chickenpox. They don't routinely give that shot in the UK.

No flu shot because it's not flu season right now.
