Normally, a K-1er simply submits their DS-3025 to the USCIS along with their adjustment of status packet. However, there have been a few reports here on VJ where some people got an RFE when they submitted only their DS-3025. In those cases, the USCIS wanted the DS-3025 transcribed onto the I-693.

If you want to play it safe, submit an I-693 along with the adjustment of status packet. All you need to do is to have a civil surgeon transcribe the information on the DS-3025 onto the I-693, the civil surgeon signs the I-693, and the civil surgeon seals the I-693 in an envelope. That's what Chinook and I did. This process cost us about $35 and took about 15 minutes in the civil surgeon's office.

By the way, I think there are circumstances where another medical exam would be necessary. For example, if the St. Lukes medical is over one year old, I believe you would have to have another medical exam.

What do you mean by documentation that she is "clear?" Clear of what?

She does't need to go to the doctor again unless she had TB issues at St. Luke's. Just go get the DS-3025 transcribed onto the I-693. The most difficult part of this process is finding a civil surgeon who knows the drill for K-1ers. Many civil surgeons want you to get more shots or another medical. I hope you don't have any trouble finding one who knows what to do for K-1ers.
