So we are filling out the form at the moment. And we hadnt even noticed we need a "vaccination supplement" until now. I just saw a thread with a similar question, but dont want to take it over with mine...

How are we supposed to do form I-693 without any evidence for the civil surgeon that he (my now husband) already had a medical before coming to the US? He had to do it in order to get the K1 visa. And then the doctors sealed the information. So obviously he couldnt get a copy of all this. We wanted to send the entire packet in the next couple of days. But now we are worried about that missing information. Can the civil surgeon get a hold of the information that the consulate and immigration officers received? Can we just make a note in the packet that the medical was done overseas in order to get the visa and have no copies?

Please help!!!

Did he not receive a copy of the DS-3025 vaccination worksheet at his K-1 medical? Did he get all the required vaccinations at his medical? If he did not have all the required vaccinations at the time of the K-1, then he needs to see a civil surgeon to get the missing vaccinations and have them fill out page 1 and 5(vaccination sheet) of the I-693. You will need to call around until you find a civil surgeon that won't insist on giving another medical and/or charging for one.

Since your husband was granted a K1 Visa he does NOT need another complete medical check, just the vaccinations part, because they are not mandatory for this type of visa (for most of the countries) and therefore, need to be taken care of for AOS. That means they will ask for official proof (most likely in English) from his doctor back at home stating which shots he's taken. They would then revise it, give him the ones he's missing (if any), and sign the supplement page. However, as Jay-Kay says, many of the authorized civil surgeons won't want to do just the vaccinations part, and try to rip you off by making you pass a pricey complete medical check.

Check the list of civil surgeons on the USCIS website and start calling and asking for prices. Insist that you're a K1-Visa and only need the vaccinations supplement.
