You can submit your AOS without the I-693 transcription. However, USCIS may or may not RFE your for it.

The OP in the above link got interview appointment without RFE.

I'm just worried that having an RFE will slow down my application as a whole, Im getting pretty desperate to work and im not sure if it would be better to wait until i can get my I-693 completed or send in my complete package minus the medical transcript and at least get my case in motion, and even if i do get an RFE my case is already being looked at so maybe it wont be so bad ?

Lewis,

I am sorry I did t see this post until today. Your form is a mess and was not properly filled out so definitely should have had an RFE. Was this done by Knightsbridge in London? In my opinion when USCIS says "if your DS-3025 was not properly completed...", I think that means properly filled out as well as shots done.

You had 3 doses of DTP but the last one was in 1990(?). It's hard to read. It needed to be within the last 10 years so you should have gotten a Td. Yet they put a check mark in Completed column and wrote n/a for K1. No, it wasn't completed and NO, they are not instructed to make notes about your visa type. They are to mark complete, or a reason for waiver, or leave it blank if you didn't get a shot.

Your M, M, and R. Not sure if Mumps requires 2 doses. I would have to look that up. But the check marks are in the column indicating the shot was given by the panel physician. And personal notes again!

Varicella is perfect.

Influenza was not given. It should have a waiver column marked which is the last column "not flu season". Instead they put a check mark in "completed." No, you didn't actually complete it, but it has a reason to be waived. Again personal notes saying n/a for K1. Everything is n/a for a K1, but the point is they should mark the shots or reason for waiver and stop commenting on visas or trying to create their own form by adding a checkmark and comment

Yes, It was Knightsbridge who did this, I thought it was kind of a mess myself but i just assumed they did it like this for everybody haha, I went ahead and got the medical done again completely, and also the neccessary vaccines i needed, after the whole medical and all injections it came out at just over $300, so i'm not really mad about it, at least now if i get an RFE it shouldnt be because of my medical. Getting the I-693 in the sealed brown envelope seemed a bit more legitimate than that scrawled on piece of paper from Knightsbridge haha.
