Hello,

Anyone who have gone through the application of adjustment of status I need you guys to share your experience about having the form I-693 filled out by a civil surgeon. I understand that since my oversea medical examination for K-1 visa is less than a year prior to applying for adjustment of status, I am not required to repeat the whole medical exam again but to submit the vaccination part. Now on the I-693 form, there are five pages, I'd like to know which pages need to be filled out.

If you had all the required vaccinations at the time of your K-1 medical and they are all marked off on a properly filled out DS-3025, then skip the civil surgeon and just send a copy of the DS-3025. If you are missing vaccinations, then you will need to call around to find a civil surgeon that won't insist on a new medical and/or charging you for one. As per the I-693 instructions, you will need page 1 and the vaccinations sheet(page 5) filled out by a civil surgeon if you are needing any of the vaccinations still.
