Timeline

The place you go through immirgration. Most likly it will not be TN. I assume you will fly from Philippines to Tn? So when you first land in the U.S. you will have to go through immigration. Depending on your flight arrangements it could be Detroit, San Francisco, or a few others.

Thank you but how do I know about my POE?Also Im trying to fill out St. Luke's medical form online,but I don't have a home fone number,Put "none" on the field but still I'm getting an error message and can't register.

regarding your POE.. it will be POINT OF ENTRY. it will be depending on the ticket you purchase. it will be the first airport within the USA soil you arrive at....

for example:

if you prefer a direct flight. best bet is MANILA-LAX (VIA PAL) then connect to TN. LAX (los angeles) will be your point of entry. you will need to pass thru immigration in LAX then connect to your flight going thru TN.

It really doesn't matter what you put for POE, its only for St. Luke's has nothing to do with anything beyond there. Put Tennessee down, even though it won't be your POE, as for phone number, use your cell number.

For future reference, your POE is considered when you first touch down in a US Government run facility whether it be an airport in a US Territory like Guam or when you first touch USA(depending on your flight schedule). Some people make the mistake between customs and POE, which can be at the same location or seperate depending on your flight schedule. In my wife's case her flight was from Manila->Guam->Honolulu->San Francisco->Home city in CA . Her Point of Entry was Guam(K1-Check/Addition of I-94 to passport), yet customs check occured in Honolulu(check/recheck baggage/official entry to USA).
