Hey guys , i am applying for AOS through marriage . following are my visa statues since 2006

I came to US on F1 , after that i got OPT (EAD) two times , first time 12 months and the extension 17 months . After that I am on H1b (current Status) .

my Question is what do i have write for question no 11 in I 765 , do i have to mention both the first time EAD date , extension date ? ( i only have the approval receipt for Extension)

Also do we have to send marriage pics , lease documents and any other proof with the application package ?

Moved from adjustment of status from family based visas to adjustment of status from work student tourist as the OP entered on an H1B work visa.

Your interview took 67 days from your I-129F NOA1 date.

AOS was approved in 2 months and 8 days without interview.

I would include details of your most recently-issued EAD on Q11.

The H-1B is not really "employment authorisation from USCIS", it's a visa and status that already allows you to work, so I personally would not mention that, other than to say that H-1B is your current, valid, status.
