The instructions for !-485 state If you are a K-1 fiancé(e) or K-2 dependent who had a medical examination within the past year as required for the non immigrant fiancé(e) visa, you only need to submit a vaccination supplement, not the entire medical report. So I contacted a couple civil surgeons in the area, and one doesn't know what the vaccination supplement is, and the other said that they can't fill out the supplement because they didn't do the medical. What do I do? Do I go ahead and get another entire medical done, just so I can get the vaccination supplement?

Hi- Yeah, there have been lots of people who have had trouble finding a dr who will fill out the vaccionation supplement without having the medical done, and some finally do it but for a pretty hefty price... I guess keep trying and call other civil srgns in the area, although I do know that some people have submitted their old vaccination supplement from the K1/2 visa medical (DS-3025). Most pple do eventually get it done though and give that in during the AOS interview.

I would advise to keep calling or make an appt and take the instructions, copy of your K1/2 visa, and vaccination supplement and try convincing the doctor that all you need is for them to fill out the form, not the whole medical.

Have you had all off the necessary vaccinations already, or are any that you haven't had medically inappropriate for you? And if so, does the DS-3025 Vaccination Documentation Worksheet that the Panel Physician filled out for your visa medical exam show this?

If the answer to both questions is 'yes' then you may get by with simply submitting a copy of the DS-3025. Many have, many have not. Worth a try if you're having trouble finding a co-operative Civil Surgeon.

I've taken a copy of my vaccination sheet from Canada to USCIS dr. and he just transferred it on the sheet in 5 minutes.

We took the instructions from the I-485 to explain that the full exam wasn't needed. If the civil surgeon doesn't know what a I-693A is I wouldn't be using that doctor.
