I called NVC this morning to check if IV package has been reviewed. The lady from NVC told me that there is a checklist for the DS230 Part 1, #30. Instead of entering the dates, my parents put "N/A". So, the lady told me to submit a new form and fill out the dates. I also asked if I can already mail the checklist today even if I havn't rec'd the checklist email yet, and she said it's fine as long as I will include the cover sheet. And I did what she told me. I downloaded the bar coded cover sheet from the payment portal that says: ATTN: CMR. Then mailed the docs before 5:00pm today. Few mins ago, I rec'd the checklist email from NVC and it has the cover sheet that says ATTN: DR. It was also stated that I have to write the NVC Case number on the upper right corner of the document, but I did not do it, since the lady told me to just include the cover sheet and that's all. Will this be a red flag?

Once the cover sheet with the barcode is included that will be sufficient. :star:

If so, your mom needs to fill up & sign the Part I (Page 2) of the DS-230 again. I'm surprise that the lady from NVC didn't point that out to you.

I almost had my parents sent me extra (blank) DS-230 forms with their signatures too. But then there is that Part II #44. Were you assisted in completing this application. My sister assisted them there in the Philippines. I didn't want to risk filling up the form by handwriting the answers without it appearing obvious that I did it here in the states.

No, it won't be a red flag, NVC and Consulate agents are pretty good at completing missing information. Anything pending, that does not affect the depth of the case or the elligibility can and will be fixed at some point before or during the interview.
