also,your missing for ds3025.

thanks for your answer...we chose not to apply for AP due to some posts ive read of some people having a hard time at reentry. we are late filers by the way. almost 2 years. she overstood that 6 month time period that is required for AP. i did read about that ds3025 i dont believe we ever got that in Ciudad Juarez, Mexico. I suppose it was in the sealed package given to her at DHL that was given at the POE in Houston. We are in Chicago. Will this be an issue. I think they use the ds3025 to fill out the I693?? If i dont have it, how can i get it? im a little confused

Since you are filing for EAD with your AOS, why not also file for AP at the same time? There is no additional fee if filed concurrently with I-485 (as you are already doing with the EAD). All you would have to do is fill out the I-131 application, provide two more photos (for a total of six). The reason that I mention it is that due to recent prolonged wait times for AOS processing at the service centers. There are recent threads where folks that are waiting for AOS processing, have realized the need for emergency international travel and have had to go back and send in the I-131 application and paperwork, and then anxiously wait for their AP document. (If you do apply for AP later, you have to send in a copy of your I-485 NOA along with the rest of the I-131-required paperwork/docs to avoid paying the fee.)

if i may add im looking over her I693 copy from the dr and it on the vaccination record portion all vaccines were infact transferred over and at the bottom the result portion has a check mark beside X VACCINE HISTORY COMPLETE FOR EACH VACCINE, ALL REQUIREMENTS MET...DOES THAT MEAN ANYTHING?

If you have a completed and sealed (by a US-based Civil Surgeon) I-693 envelope, you should be good to go. I believe, that the Civil Surgeon should have given you a copy of the completed Form I-693 for your records.
