Seeing as how I had my medical done for my K1 two months ago, I am not required to get another medical done for AOS.

Do I HAVE to see a civil surgeon to get my vaccination worksheet transcribed to the proper form (vaccination supplement I-693A), or can I save money on this, and just send in the worksheet given to me by the panel physicial in Canada? Has anyone done this and not receive an RFE?

If I do have to see a civil surgeon here, anyone know where I can find the list of these panel physicians in the US?

You do need to see a civil surgeon. See this link:

I did not see a civil surgeon, I sent the vaccination supplement I-693A and was approved.

I was informed in the AOS forum that other people did not see a civil surgeon, and merely handed in their DS-3025 Vaccination Worksheet from the visa medical exam, and did not get an RFE.

I will most likely try this route.

Yes many of us have. Save a buck and if they RFE you then go.

I never did the supplement sheet- just sent my pack from the previous doc in Canada. Just got my appt and biometrics today with no RFE. Sweet, good luck.

The panel doctor in Saskatoon wouldn't give me the I-693A - did you have to argue with them to send it to you? Did they fax it to you or did they mail it - should I talk to the younger girl or the more adult one that is the receptionist? Tips would be great, thanks!

yep I had to argue. She kept saying they have never done it before. Tell her you know they just faxed a copy of it to me (cristy) in Arizona in September. That should stump her lol. I think it was the older one I talked to.

I sent in my vaccination worksheet, since that was all I had. I did not have a problem, but prepared if they send you an RFE, it's becoming more commonplace.
