Supplement Forms for G-325A and I-129F

There are a few boxes on the forms that I need to include more information than fits in the space. When I use a supplement form, should I only include one question on each page? Or can I write the name of the form at the top ("Supplement for form G-325A") and list all the questions and answers that I need to include information for below?

Also, I could fit my fiancé's address in the space provided, but they don't write their addresses in the format on the form, so I thought it would be better to refer to the supplement form where I can write it correctly. Is this advisable or not?
